What is GDPR?

The mutually agreed General Data Protection Regulation (GDPR) came into force on May 25, 2018, and was designed to modernise laws that protect the personal information of individuals.

The GDPR outlines six data protection principles practices must comply with when processing personal data.
These principles relate to: Lawfulness, fairness and transparency - we must process personal data lawfully, fairly and in a transparent manner in relation to the data subject.

This easy to use patient questionnaire has been validated for use in Primary Care.

It is used by your doctor to assess your level of physical activity.
