HH Collaborative PCN was formed by 8 GP practice within the Hayes and Harlington area.
We work together and with other health care providers within the community including doctors, nurses, pharmacists, practice managers and other non-clinical staff to standardise care across the practices and provide our patients with efficient, streamlined services.
We also work in partnership with the Hillingdon Council for health promotional work such as weight reduction and smoking cessation.
Our workforce is engaged in regular training and improvement.
